You do not need to complete this form if you paid your bill or invoice by Direct Debit - we will automatically use the Direct Debit bank details to refund the credit to you. However, if your bill/account is in credit and you have not received a refund, please complete the Refund Request form.

Accounts in more than one name

If you are requesting a refund for an account which is in more than one name, you will need to upload a letter signed by all named parties confirming which bank account the refund should be sent to.

If you cannot obtain the signatures from all named parties, you will need to upload proof of payments you have made covering the full amount of credit.

If the bill payer is deceased

If the bill payer has died, executors will also be able to complete this form. You will need to supply a copy of the will or proof of a grant of probate.
